Home Health Visits
Patients are often asked to walk to local clinics to have their wound care done.
LEAD I care offers home services where we perform many skilled nursing duties such as wound care, catheter changes, and more...
SKILLED NURSING care services include:
*Injections (infusion therapy, insulin administration)
*Monitoring vital signs, including Blood Sugars
* Making referrals to rehabilitation therapists for Physical Therapy, Occupational Therapy, and Speech Therapy.
*Making referrals to Hospice Services as needed
* Medication Administration
When used properly, medicine can help control illness, relieve pain and fight infection. Medications can also cause serious problems if taken in the wrong dosage or combination. LEAD I CARE providers can help you manage your medications and ensure that you take them in a safe and effective manner. We also have a wide range of herbal medication to supplement and/or compliment medical regiment.
MEDICAL CARE COORDINATION
What is coordinated care? At LEAD, our staff works with you, your caregivers and your doctors to develop a Plan of Care (POC) designed to meet all of your individual needs. As a LEAD I CARE patient, you’ll be assigned a primary care coordinator who will manage your Plan of Care. Your coordinator will:
*Review your needs and abilities on an ongoing basis
*Keep your doctors, family, caregivers informed about your condition
*Determine whether specialists are required (PT/OT/ST)
*Evaluate your recovery
LEAD I CARE providers work with you to adjust your Plan of Care as necessary, so you continue to make progress and achieve the best possible health outcomes.
*PASTORAL CARE/SERVICES
LEAD I CARE believes in a holistic approach to wellness and we offer pastoral services. A Pastor is available to pray with you, give you Christ centered advise and counseling. We have healing scriptures to help you pray the WORD for your healing.
* ESCORT SERVICES
We will upon request escort our clients to the doctor’s office visits to advocate for them, get necessary information for them to better manage their health at home.
